Candidate Personal History Requirements

Entry Level applicants for KCSO positions are ranked on an "eligibility list" based on their written test, physical agility test, and oral board results. Once you are ranked the following information is required on a Personal History Statement (PHS). We recommend that you start gathering the information right away. The sooner you accurately complete the PHS the sooner you can be considered for hire. Entry level applicants will complete their PHS through Public Safety Testing. Lateral applicants will be sent a PHS packet by the Chief Examiner.

You need to provide a non-certified copy of your:

  • Birth Certificate
  • High school diploma/GED
  • College diploma if applicable
  • Military form DD214
  • Social Security Card
  • Driver's license
  • Divorce decree(s)
  • Bankruptcy papers

You need to supply the following information:

  • 3 Professional references - addresses, phone numbers
  • 3 Friends - addresses, phone numbers
  • Addresses back to age 15
  • All traffic infractions - type, date, location, disposition
  • All prior drug use - times and dates used
  • Vehicle insurance - company, address
  • Financial information - income, expenses, debts, liabilities, assets, credit cards
  • Jobs - employer, address, phone number, supervisor, coworkers, salary, job title, duties
  • A packet will be sent to you to help organize the data
